Arkham Asylum is officially under new management as Dr. Jeremiah Arkham comes to town with plans to follow in his uncle's footsteps and run the family 'business' so to speak. But Jeremiah has his unique take on psychiatry and isn't content with merely keeping up with status quo. To start with he decides to renovate the Asylum. Gutting almost everything from the old regime and even going so far as to burn his Uncle's personal journals detailing his decent into madness, Dr. Jeremiah Arkham has the grounds rebuilt in his own image. Basing the floor layout on an actual labyrinth and installing state of the art security equipment, Dr. Arkham works closely with his contractor Mr. Zolly Hiram in hopes of rehabilitating Arkham's image as a revolving door for the criminally insane.

Along with new floor plans, Jeremiah also brings with him several new treatment approaches for some of Arkham's most troubling inmates. This includes holo-projector scare tactics for Scarecrow, a little roughness for Cornelius Stirk and dream prevention therapy for Everard Mallitt, an especially troublesome patient who goes on about his delusions of a hero who appears to him in his dreams and offers him release in exchange for a share of Mallitt's wealth. Dr. Arkham also spends a bit of his time conversing with one Victor Zsasz, a murderer so dangerous he has to be kept in a windowless tube so as to prevent him from murdering.

But all of this pales in comparison for what Dr. Arkham has in store for his favorite patient - Batman! Charged with murder, Batman is now under Dr. Arkham's treatment for his alleged insanity. Nevertheless, Dr. Arkham refuses to remove Batman's mask, insisting that when the time is right, the Caped Crusader will divulge that information himself willingly!
